To make the enrollment process easier, families may download and complete the required forms below. Please read each form carefully and complete all sections that apply to your child.

Completed forms may be brought to your scheduled enrollment appointment. Please do not send completed forms containing private medical, contact, or identification information through the website.

Completing the forms does not guarantee enrollment. Enrollment is based on available space, completion of all required documentation, and final approval by TT’s Loving Care.

Tennessee DHS Child’s Application (Required)

This official Tennessee Department of Human Services Child’s Application is required for all children enrolling at TT’s Loving Care. Please complete all applicable sections and bring the completed application with you to your scheduled enrollment appointment. Enrollment cannot be finalized until all required forms and supporting documentation have been received.

Parent Handbook (Required)

Please read the Parent Handbook carefully before enrolling your child. It contains important information about TT’s Loving Care policies, tuition, holidays, illness procedures, emergency closings, provider vacations, discipline, daily routines, and other program expectations. A signed Parent Handbook Acknowledgement is required before care begins.

Parent Handbook Acknowledgement (Required)

This form confirms that you have received, read, and agree to follow the policies outlined in the TT’s Loving Care Parent Handbook.

Tuition Agreement (Required)

This agreement outlines weekly tuition rates, payment due dates, late fees, payment methods, and other financial policies. Both parent(s)/guardian(s) must sign before enrollment is complete.
